Limit of Services Disclosure Form:

Protect yourself and your clients – use the Limit of Services Disclosure Form in your practice to make sure your clients understand the scope of services you will be providing. Customize this form for your practice and attach a copy of your State’s specific Business and Professions Code, or any other appropriate State Code for your clients’ convenience. Keep the original in your client’s file, and provide him/her with a copy for his/her records.
