Membership Upgrade

Please follow these steps to upgrade your membership.

1. Download the membership application form and fill it out.
2. Make one copy of your diploma or certificate of training.
3. Fax or mail both documents to us. See our contact page for our address and fax number.
4. Pay for your membership using one of these methods:
   1. Use our online payment system
   2. Send a check by mail
   3. Pay by telephone
